Hi can one of the php html guru's help me out with ow to get this going in a block?

i have no idea how to put this in a block
how to peice all the codes  etc,

can you help me out putting this togeather Please would love it to work on my site.

There has to be a few of you have one of theses on your site, if i new i wouldn't be asking for help how to put the code together
