I have split your post. Please start new topics for your issues.

First, take a back-up of your index.template.php file.

Ignore the error and install the mod.

Apply the index.template.php edits manually using the manual installation instructions here (http://custom.simplemachines.org/mods/index.php?action=parse;mod=1104;attach=120231;smf_version=1.1.10).

At this point, if you see a problem, just replace your index.template.php file with the one you backed-up at first step.
